Job Description
Provides individual and/or group social work services to patients and families in healthcare settings using psychosocial assessment skills.
Responsibilities
Provides culturally competent services; identifies patients needing assessment/intervention; advocates for access to care; communicates assessment findings; provides psychoeducation, counseling and crisis intervention; develops treatment/discharge plans; connects community resources; documents care; may supervise social work students.
Qualifications
Master of Social Work required. Current New York State Master Social Worker or Clinical Social Worker license required plus specialized certifications as needed. 1-3 years relevant experience required.
